Obtaining application

Registration of graduate physicians (BAMS / BUMS / BSMS)

  • Fee for the application is Rs. 250.00 - (Postage to obtain by post is Rs. 85.00)

Registration of physicians with Diploma (DA / DAM) (Ayurveda / Siddha)

  • Fee for the application is Rs. 250.00 - (Postage to obtain by post is Rs. 85.00)

Registration of hereditary physicians - 55 (1) (E) - General Physicians/ 55(2) Special Physicians.

  • Fee for the application is Rs. 250.00 (Postage to obtain by post is Rs. 85.00)

Submission of completed application

Registration of graduate physicians (BAMS / BUMS / BSMS)

  • A certified photo copy of the original birth certificate.
  • Certificate of pass issued by the relevant university
  • A photo copy of the National Identity card.
  • The medical council form 01 /05 issued by the Ayurvedic medical council.

Registration physicians with Diploma (DA / DAM) (Ayurveda / Siddha)

  • A certified photocopy of the original birth certificate.
  • The final certificate of pass issued by Ayurvedic Department .
  • A photocopy of the National Identity card.
  • The Medical council form 01/05 issued by the Ayurvedic medical council.

Registration of doctors hereditary physician 55 (1) (E) - General physician / 55(2) - special

  • Every applicant should send his birth certificate (original copy) along with the application .
  • In case of Buddhist months they should sent the upasampada certificate along with birth certificate or an affidavit containing the lay name along with Buddhist Bhikku identity card issued by the Ministry of Buddhist affairs(a certified copy should be sent)
  • Medical council No. 01/01 - Hereditary teacher’s medical certificate.
  • Medical council No. 01/02 - Certificate of experience issued for being a practitioner in hereditary medicine(should be certified by any two of the following persons a Justice of Peace/ Viharadhipathi / Principal of a Govt. Collage)
  • Medical council No. 01/03 - Divisional secretary’s certificate.
  • Medical council No. 01/04 - Personal particulars of the applicant .
  • A certified photocopy of the National Identity card.

Issue of identity cards to registered Ayurvedic physicians

(The relevant application form could be obtained from the Medical council office free of charge)          

  • A certified photocopy of the certificate of registration
  • Identity card fee of Rs. 1000.00

Renewal of the identity card already issued.

  • Issued identity card.
  • Identity card fee of Rs. 1,000.00

When the issued identity card is lost.

  • A copy of the complaint made to the police regarding the loss of the identity card.
  • Identity card is Rs.1,500.00

Inclusion of additional qualifications of the registered Ayurvedic physicians

(The relevant application form could be obtained from the Ayurvedic Medical Council office free of charge)

  • A certified copy of the certificate indicating additional qualifications to be included in the register.
  • A photocopy of the registration certificate in the Ayurvedic Medical Council.
  • A photocopy of the National Identity Card.
  • A photocopy of the certificate issued by the relevant institution to certify the discipline in which additional qualification was obtained.
  • The fee for issuing a Substantial Sinhalese copy of certificate inclusive of the additional qualification is Rs. 3000.00 - (Rupees Three Thousand)
  • The fee of issuing both Substantial certificate in Sinhala inclusive of additional qualifications and a copy of it’s English translation is Rs. 5500.00 - (Rupees Five Thousand and Five Hundred )
  • Postage of Rs. 100.00 - for obtaining the certificate by post.

Issue of copies of registration certificate of Ayurvedic physicians Issue of a copy with alteration of name

  • Amended birth certificate.
  • A certified copy of the amended National Identity Card.
  • The form A.M.C. / G/ 04 Issued by Ayurvedic Medical council free of charge.
  • Fee for alteration of name is Rs. 2,500.00 - (Postage to be sent by post Rs. 100.00)

Issuing substantial Sinhalese / Tamil certificates

  • A copy of the complaint made to the police in the loss of registered certificate.
  • The affidavit to the effect that the registered physician is himself under the relevant registration no.
  • Form A.M.C / Gen / 04 issued by Ayurvedic Medical council free of charge.
  • The fee for issuing copies if substantial Sinhala / Tamil certificates is Rs. 2,500.00 - (Postage to be sent by post is Rs. 100.00)

Issuing copies of English certificates

  • Photocopy of the certificate of registration
  • Fee for issuing copies of English certificates is Rs. 2,500.00 - (Postage to be sent by post is 100.00)

Issuing international certificate

  • A certified copy of certificate of registration in the Ayurvedic Medical Council.
  • A certified photocopy of the identity card issued by Medical / council.
  • A photocopy of the National Identity Card.
  • Fee for a copy of international certificate is Rs. 10,000.00 - (Postage to be sent by post is Rs. 100.00)

Issue of medical certificate books to registered Ayurvedic physicians

For Personal Callers

  • Medical Certificate Books by the medical council.
  • Cost of per Medical Certificate book is Rs. 350.00

Delivery by post

  • Letter of request fees the relevant physician
  • Photocopy of the identity card issued by medical council.
  • Price of a medical certificate book is Rs. 350.00 - Postage per book is Rs. 100.00

Regularization and control of professional behaviors of Ayurvedic physicians, Ayurvedic dispensers and Ayurvedic nurses

Refer to preliminary investigations

  • On receipt of a legal complaint against a registered physician a preliminary investigation will be conducted according to the approval of the council.

Refer to the Committee of Penalties

  • Accused persons ad decided by the Council on the Preliminary Investigation Report will be referred to the Committee of Penalties.
